I understand that photographs and/or video recordings may be taken during the Titanettes Ice Girls Skills Clinic and related game-day activities.

I grant permission to CMXI•NEXT Productions, Titanettes Ice Girls, NJ Titans, and their affiliates to use my child’s likeness, image, or voice for promotional, marketing, social media, and website purposes without compensation.

ASSUMPTION OF RISK & LIABILITY WAIVER

I acknowledge that participation in the Titanettes Ice Girls Skills Clinic involves physical activity, including but not limited to dancing, movement, and participation in a live sports environment.

I knowingly and voluntarily assume all risks associated with participation, including injury, illness, or accident, whether caused by my child, other participants, facility conditions, or otherwise.

I hereby release, waive, and hold harmless:

Arena ownership and staff
Event sponsors, partners, contractors, volunteers, and affiliates

from any and all claims, liabilities, demands, or causes of action arising out of or related to participation in this event, whether caused by negligence or otherwise, to the fullest extent permitted by law.

MEDICAL AUTHORIZATION

In the event of an emergency, I authorize CMXI•NEXT Productions and its representatives to obtain medical treatment for my child if I cannot be reached.

CODE OF CONDUCT ACKNOWLEDGEMENT

I understand that this is a professional, structured event. Participants are expected to follow instructions, behave respectfully, and adhere to safety guidelines. Failure to do so may result in removal from the clinic without refund or liability.
